🚧 What are we working on?
Current priorities – IMTP 2020–2023
Our current focus includes improving care for frail older people and reducing the risk of falls across the network.
✅ What we have already done
- Developed a falls prevention guide for the Cwmtawe area, with support from SCVS
- Introduced a nurse prescriber–led system for reviewing continence needs
- Established parent and carer forums
🔮 Future priorities for the cluster
Future work will focus on addressing polypharmacy issues identified in previous cluster plans.
