Data Protection and Your Medical Records
🔐 The practice complies with data protection and access to medical records legislation.
Identifiable information about you will only be shared in the following circumstances:
- To provide further medical treatment, for example from district nurses or hospital services
- To help you access other services, such as social services (this requires your consent)
- When there is a legal duty to share information, for example in child protection cases
- Anonymised information may be used locally or nationally to help plan NHS services (this data does not identify you)
If you do not wish your information to be used in this way, please let the practice know.
📁 Welsh GP Record
The Welsh GP Record is a summary of important information from your full GP medical record.
This summary can be viewed by selected healthcare professionals in Out of Hours services and certain secondary care settings.
Your Welsh GP Record includes:
- Your name, address and contact details
- Current medication and medication prescribed in the last two years
- Allergies or known adverse reactions
- Current problems or diagnoses
- Results of recent tests from the last year, such as blood tests or X-rays
It does not include any discussions you may have had with your GP.

🗂️ Management of records
Records and information management are key parts of the NHS Information Governance framework.
Good records management helps ensure the right information is available at the right time to support safe care and informed decision-making.
The practice manages records in line with:
- UK General Data Protection Regulation (UK GDPR)
- Data Protection Act 2018
- Freedom of Information Act 2000
Records covered include administrative records, paper and electronic documents, emails, audio and video recordings, X-rays and CCTV footage.
